To your convenience, you have an option to pay your life policy premium via following methods;

Salary Deduction (Top)

Salary Deduction via Biro Perkhidmatan Angkasa
This payment method is applicable for our policyholders who work with government agencies.  Simply submit the completed BPA 1/79 form to authorize the deduction of your salary.

Salary Deduction via Accountant General
This payment method is applicable for our Etiqa Insurance Berhad’s policyholders who work with government agencies. Simply submit the completed Salary Deduction Authorization form to authorize the deduction of your salary.

Salary Deduction via Employer
This payment method is applicable for our policyholders who work in private companies. Simply submit the completed Salary Deduction Authorization form to authorize the deduction of your salary.

Auto Debit (Top)
Your premium payment will be automatically deducted from your saving or current account of the following participating banks;.

o Maybank

o CIMB

o Bank Simpanan Nasional

You can start enjoying the facility by submitting the completed Auto Debit Form to us.

 

Bank Standing Instruction
You may make instruction to your bank to pay your policy premium via regular debiting of your bank account. Application for Bank Standing Instruction is available for all banks. Visit your nearest bank and complete the Direct Debit Authorization Form and submitted to the Bank

Credit Card (Top)
You can pay your policy premiums using your credit card (Master/Visa)  through:-

o Point of Sale - You can use your credit card to pay your policy premium at our branch network.

o Auto Pay - By submitting completed credit card deduction authorization form to us, your policy premium will be automatically deducted from your credit card

o EzyPay - You can opt to pay your policy premium in free interest installment plan via ezypay from Maybank visa & mastercard.

 Internet Banking (Top)
Give you the freedom and flexibility to manage your personal and business finances quickly and easily 24 hours a day, 7 days a week. Premium payment can be made through the following internet channels.

 Cash or Cheque (Top)
You may make payment via cash or cheque at ;

· Public Bank Berhad

· Bank Simpanan Nasional

Alternately, you may also mail your policy premium cheque payment to Etiqa HQ. Pls ensure that you have written your name, Identification number and policy number at the back of the cheque.

 

 

Claim FAQ (Top)

· Who can certify claims documents?
These are the people who are allowed to certify documents:-

o Commissioner of Oath

o Police

o Lawyer

o Doctor

o Branch personnel (executive and above)

o Employer

o Penghulu

o Agency Head

o Agents

o Government staff (Division one)

o HQ staff (executive and above)

· What is contestable period?
The time limit during which an insurer may contest or challenge the validity of the policy due to fraud, suppression of material facts or misrepresentation in the application.

· Do I need to submit the medical report for 2nd installment of Total and Permanent Disability (TPD) claim?
Yes. The medical report on Insured’s current medical condition is necessary to proof continuity of disability. In the event that the covered person ceases to be totally and permanently disable, the Company shall discontinue any further installment payments or waiver of premium, where applicable.

· I have submitted the TPD claim. Do I need to pay my premium which is going to due next month?
Yes, it is advisable to pay the premium until the claims has been approved. Excess
premium will be refunded to you, if any.

· Why do insurance company requests for `Letter of Administration’ from claimant?
Where a policy owner dies without having made a nomination, the Grant of Probate or Letter of Administration or Distribution Order will be required to distribute the policy moneys accordingly.

· Where can I obtain the claim form?
Claim form is available in our Branches and Customer Service Centre. You may contact your servicing agent or our customer service at 1-300-13-8888 for assistance.
